Igor  Buzaev : Hello!

Irina Pershina : Doctor, what is Parkinson’s disease?  

Igor  Buzaev : Parkinson’s disease is  neurodegenerative  disease. Cells that in  produce dopamine in the brain –  mediator that transmits information from one  cell  to another,  begin to die. As a result, the transmission of information  from one cell to another begins to suffer, and this causes such symptoms,  such as stiffness, slowed movements, tremor, muscle rigidity. Currently, in Parkinson’s disease, medications are usually prescribed that replace this dopamine, deliver  его  into the brain  But unfortunately, these medications over time begin to weaken in their effect. There is even one study that  microbiome  a person uses dopamine from these medications  for food, which is why the substance does not reach the desired place. Therefore, in addition to conservative methods, surgical treatment methods are used, among which deep brain stimulation can be highlighted  Thalamotomy  gamma knife and radiofrequency ablation. Kirill  Radzig : Here’s the method we’re talking about today, how is this conducted  operation,  if it can be called that?  

Igor  Buzaev : Since we provide enough  strong mechanical action  on tissue,  it’s called surgery,  performed exclusively by a neurosurgeon  The essence of the method is that a special helmet is placed on the person’s head. The helmet contains 1024 ultrasound transmitters. Then, the patient with the helmet is directed to an MRI.  The neurosurgeon marks the target points on MRI images where the ultrasound is then focused. The ultrasound is focused  in a small point near 1  мм.,  the accuracy of the installation is 0.1 mm  Further, step-by-step heating is performed  first, we can heat the neurons to a state where they  «turned off, but not dead  and were not damaged. I changed my field to dedicate myself to it.  

Irina Pershina : What happens after the treatment ends? We have seen that a person’s tremor disappears. But can it  return again? Can other areas be affected? After all, Parkinson’s disease is still incurable  

Igor  Buzaev : При  Parkinson’s disease deficiency  dopamine is not eliminated. And therefore, of course, the disease may progress. Medication is definitely required  medications containing dopamine. But we significantly  improving quality of life. And for most patients, the effect is quite lasting, more than a year.  We started working in May 2020. A year and a half  we  observing  our patients  и  see enough  good results  

Irina Pershina : How do you think  is this method the future? Perhaps, in a few years, exactly  he  replace all the others?  

Igor  Buzaev : In the 90s, when I began my medical career, endoscopy was an exotic procedure. Now  people almost all operate  endoscopically. I see the same thing now: focused ultrasound  can  become routine. Now we have started with  essential  tremor, then  – Parkinson’s disease. Then there will be epilepsy, tumors, inaccessible  surgeons in complex localizations. There are studies that  now started on Alzheimer’s, there is a slightly different mechanism of action. We see that this method is gaining momentum everywhere. In addition, the method  is already used for the treatment of prostate cancer, uterine fibroids, bone tumors  This becomes the surgery that  is extremely precise and controlled.
